Here is how to book an appointment

For all new patients to our clinic, you MUST book the initial examination option so that we will have time to perform a compressive examination and treatment.  ​

New Service:  Because of the high number of patient treatment requests, and limited treatment spots.  We are considering opening additional holiday hours.  This could be evenings or Sundays.  This service will ONLY be open to existing patients.  If interested please email Dr. Jason Mazzarella.  You will be put onto a list, and when new hours open, you will be included in the email chain.  These additional booking hours will be scheduled by Dr. Mazzarella, not through the online system.
